You must print and send us the form, you can not fill it in online.
Or call us at 262-227-4717 or 262-227-0595 and we can register you over the phone with you mailing a check or money order after your call.

After making those selections click one of the buttons labeled "View Cart". You will be in a shopping cart page provided by Pay Pal. You can change the classes or your order there further - it allows you to change your mind on anything. When you are done click the "Proceed to Checkout" button to use your charge card to pay for the class.
PayPal accepts VISA/MC and others - (you don't have to join PayPal), and when you complete the payment we will automatically have you registered wih your address and your e-mail.

Or you can choose the Mentoring Course Standard Arrangement (as described on the Mentor Course page) and register here than email or call to arrange the time and date to meet us and discuss the shedule you want to use.
This arrangement is 20 hours of personal one on one instruction and 5 more hours of critique and training online.
You can start with a trial lesson that is two hours minimum at $ 75 an hour also planning it for a date and time convenient for you. Resister for it then call or email to set up the time.
Lesson locations will be Dattilio Studio, 1820 N. 166th St., Brookfield and other places determined by your project needs

NOTE: You cannot fill out this form online and send it to us. It must be snail mailed. But if you prefer to use an online registration and payment system use the online shopping cart on this page there is no need to send in the printed form. We will be sent all your information through the shopping cart system. However when you check out with the shopping cart you will have to make your payment with a charge card using PayPal. This is very easy and you don't have to join PayPal. It works the same as placing a charge card order at any online store. It is also one of the most secure online ordering systems in existence.
